Policy Ambassador Application

Thank you for your interest in being a Parkinson’s Policy Ambassador. The Michael J. Fox Foundation (MJFF) and our Ambassadors will work together to ensure government policies improve quality of life for people with Parkinson’s and their families. As an Ambassador, you will work to make a difference by being a champion, leader and mentor in your community. You will help us ensure that members of Congress hear directly from their constituents to help shape their decision-making.

If you have any questions about this application, please email sweissmann@michaeljfox.org.

We ask for the information below because sometimes Congressional offices want to talk to people that meet certain demographic criteria.

Recent Advocacy Work

We would like to get a sense of your advocacy work, both on behalf of the Parkinson’s community and beyond. We can see your activity in responding to action alerts sent from The Michael J. Fox Foundation but we are interested in other activities you may have participated in.

Community Involvement

We would like to get a sense of your community involvement, both political and non-political.
